General Medical Insurance Information & Eligibility for SHAC Clinic Services


Please note that claim forms are not required. To submit a claim, make a copy of the bill/receipt, write your name, medical ID number, PSU ID number, and what university you attend directly on the bill/receipt. Submit to:

For Fall, Winter, Spring, and Summer terms:

  • Students enrolled in 5 or more regular In-Load* credit hours are automatically assessed a mandatory $177 Student Health Fee, $63 of the $177 is a Basic Insurance Plan. The Basic Insurance Plan offers coverage for students who need services outside of SHAC. It also helps cover the costs of diagnostic work, such as labs or x-rays, performed at SHAC.

  • Students who have paid the Student Health Fee and therefore have the Basic Insurance Plan may voluntarily enroll in the Supplemental Insurance Plan (a more traditional "major medical" insurance plan), for an additional fee, within the Add/Drop deadline of each term by calling Wells Fargo Student Insurance Services at 1-800-853-5899.

  • DROPPING CLASSES: Any student who drops below 5 credit hours within the first four weeks of any term will lose access to the Center for Student Health and Counseling and their Basic Insurance Plan. Any claims accrued during this time will be the responsibility of the student.

*In-Load credit hours are those credits that are not considered Self-Support. Self-Support classes are not assessed student fees in the same manner and do not count towards eligibility for health services and health insurance.
