the bill/receipt, write your name, medical ID number, PSU ID number, and what university

For Fall, Winter, and Spring terms:
Students enrolled in 9 or more regular In-Load* credit hours are
automatically assessed a non-refundable Student Health Fee, which includes a Basic
Insurance Plan. This fee allows students to use the services at the Center for
Student Health and Counseling.
Students enrolled in 4 to 8 regular In-Load* credit hours have the option of
voluntarily paying the Student Health Fee. This must be done by the Add/Drop deadline
of each term. Please see below for the enrollment deadlines.
Students enrolled in 1 to 3 credit hours are unable to pay the Student Health
Fee and unable to use SHAC services.
Students who have paid the Student Health Fee and therefore have the Basic
Insurance Plan may voluntarily enroll in the Extended Insurance Plan, for an
additional fee, within the Add/Drop deadline of each term by calling Wells Fargo
Student Insurance Services at 800-853-5899. See below for enrollment deadlines.
DROPPING CLASSES: Please be aware that if a student drops below 9
credit hours it is the student's responsibility to make sure that their Student Health
Fee is retained. A student may do so by going to the Cashiers window in Neuberger Hall
within the Add/Drop Deadline to ask that their Student Health Fee be retained.
After the Add/Drop deadline has passed this is no longer possible.

*In-Load credit hours are those credits that are not considered Self-Support.
Self-Support classes are not assessed student fees in the same manner and do not
count towards eligibility for health services and health insurance. To read the

During the Summer term:
Summer term eligibility is based on Spring term eligibility or Summer term enrollment.
Students enrolled in 1 or more credit hours for Summer term are
automatically assessed a non-refundable Student Health Fee, which includes a
Basic Insurance Plan. This fee allows students to use the services at the Center for
Student Health and Counseling.
Students not taking any Summer classes may access SHAC services on a
Fee-for-Service basis if they were SHAC-eligible in Spring (see above for eligibility
criteria). Fees range from $10-$50 depending on the type of visit you have.
Diagnostic charges incurred for those not taking Summer classes will also be
the student's responsibility unless the Basic Insurance Plan is purchased. Students not
taking any Summer classes, but who were SHAC-eligible in Spring may voluntarily enroll
in the $40 Basic Insurance Plan for Summer by calling Wells Fargo Student Insurance
Services at 800-853-5899. See below for enrollment deadlines.
Note: Students who are enrolled in 1 credit hour or more for Summer
term, and therefore have the Student Health Fee which includes the Basic, or who
were SHAC-eligible in Spring term and enrolled in the Basic Insurance Plan for Summer
term, may also voluntarily enroll in the Extended Insurance Plan.
